The Patient Portal is a secure, confidential, and easy-to-use website that gives patients and their families 24-hour access to their medical records.

Adelante Healthcare is using NextGen, one of the leading Patient Portal platforms in the U.S.

It uses the latest encryption technology to deliver secure communication between patients and providers.

By signing up and enrolling with the Patient Portal, you will be able to:

  • Schedule and cancel appointments
  • Make payment to your account
  • Receive appointment reminders
  • Access important health information from your medical records including immunizations and your list of medications
  • View lab test results
  • Vide health education information
  • See the patient plans from your office visits

How do I retrieve my user name?

Click on the need help with user name and password link and follow the instructions. A link to reset your password will be emailed.

How do I retrieve my password?

Reset the password by entering the user name and answering the security question. A link to reset your password will be emailed.
Your doctor’s office also can help you reset the password by providing a link and a password reset token.

Can I use the Patient Portal on my mobile device?

Yes, the NextGen Patient Portal is now available for mobile devices and tablets.
There is no special app to download, just open your enrollment email and click Patient Portal link or enter the link in your mobile browser to access the mobile site.

Can I book appointments from the Patient Portal?

You can book appointments online using NextGen Patient Portal as well as send an appointment request to the practice if you are unable to book an appointment.
In addition, you can reschedule or cancel appointments through the patient portal.

Can I see lab results on NextGen Patient Portal?

Documents and lab results are automatically uploaded to NextGen Patient Portal once your provider has reviewed them.
You will recieve an email notification letting you know when a document is available in the inbox.

Can I add care managers and dependents?

You can add a care manager to manage the selected patient’s NextGen Patient Portal account, add a dependent for the selected patient to manage at the time of enrollment or at the request of the patient.

How do I Sign Up?

Once you have established care with Adelante, you will receive an email with an activation code. The code allows you to log in to the portal and create your own private user name and password.


Who can Sign Up?

Patients and their legal guardians can sign up for the patient portal in accordance with the terms and conditions of the website.

Parents, we want to provide a safe and comfortable environment for your teen. That means providing privacy and confidentiality when your teen decides to talk to their health care provider about pregnancy or sexually transmitted infections, and drugs or alcohol problems.

Because our healthcare providers want to provide this type of environment, during the ages of 12-17, parents will not have access to their teen’s healthcare record through our patient portal.

We encourage our adolescent patients to talk to their parents about these difficult subjects, but we also want to provide a safe place for our adolescent patients to receive the medical treatment and information needed to stay physically and mentally healthy.

Just tell your provider who would like to enroll and you are on your way to connecting with your provider.

Who can Sign Up?

The NextGen Patient Portal is designed to protect your privacy and keep communication and medical records secure.

If you choose to use the Patient Portal, your name and email address will be treated with the same care and privacy given to your medical records.
You will receive notices to your personal email account when there is important information awaiting you in your Patient Portal account.
