Located in every state in the country, CHCs serve more than 25 million patients including more than 7.5 million children and 2.5 million Medicare patients. CHCs are sited in underserved communities where they offer cost effective health care to low income families in particular.
Their staff, from medical providers to administrative support, share a commitment to delivering the best quality care and experience to patients in their communities. Each site offers Eligibility Specialists who can help patients enroll in Medicaid (AHCCCS), SNAP, TANF, WIC, and Affordable Care Act insurance. Depending on income and family size, uninsured patients may qualify for a sliding fee scale to ensure they can afford to secure health care.
